If JVTY holds a previous waitlist, application, or interest record connected to you, use the monitored email route below. This page does not collect personal details and does not accept health information.
How the request works
- Email from a route we can verify and name the right you want to exercise: access to a copy, correction, deletion, or withdrawal of consent and future contact.
- Do not send symptoms, diagnoses, medicines, test results, government-ID images, or other health information in the first message. We will provide the appropriate identity-verification step through a separate route.
- After the case is opened, you receive an opaque reference. The operator records identity verification and every access, correction, deletion, withdrawal, refusal, or permitted extension in an immutable audit trail.
Timing and escalation
A data-rights request is due within 30 days. If the applicable rules permit one extension of no more than 30 additional days, you will be notified before the first deadline and given the reason. You may also use the official regulator complaint and report portal below; that external service does not itself execute a JVTY request.
